Please contact your Pharmacy to request your medication refill.
Your Pharmacist will review your medication regimen and send a medication refill request form to our office. Our office requires 48 hours to process prescription refills.
Your medication refill will be called directly to the requesting Pharmacy.
Please make sure you request medication refill from your Pharmacy early enough to avoid running out of your medication. Prescription refills can be considered during follow-up clinic visits, when medication intake can be reviewed by the physician, and appropriate changes to the regimen made, if necessary.
Prescription refill request placed with a pharmacy, will be considered for established patients only. For the purposes of prescription refills, an established patient is one who has received treatment at our clinic, or had surgery, in the previous 3 months. If you have not been seen in the previous 3 months, you will be required to schedule an appointment and return for an evaluation by the physician, before a prescription refill is considered.
Alternatively, you can request a prescription refill from your primary care provider. We do recommend that you visit with your medical provider, to ensure an appropriate and safe medication regimen. Unfortunately, we cannot refill prescriptions outside of normal business hours, and during weekends and holidays.
